Proctology is one of the fields that have undergone significant development in recent years. Thanks to minimally invasive methods and same-day surgery, patients can now undergo a range of procedures gently, quickly and without the need for long hospital stays. This modern approach is actively developed by the team of Július Örhalmi, MD, PhD, FASCRS, MBA at the Day Care Centre of the Rehabilitation Hospital Beroun and the Hospital Hořovice.

Wide range of procedures without the need for long hospital stays

Dr. Örhalmi's team performs an almost complete spectrum of proctological procedures in day surgery. The most common surgeries are for hemorrhoids, rectal fissures, fistulas or pilonidal sinuses.

This type of care is especially suitable for patients who prefer a short hospital stay, do not feel very comfortable staying in an inpatient ward and want to return to work and normal life as soon as possible. On the other hand, it is not ideal for patients with more serious associated diseases.

Simple preparation, a short procedure and then normal routine

"The measures before the procedure are quite simple. The patient does not eat or drink from midnight onwards, and in the morning the basic preparation of inserting a glycerine suppository is sufficient. The procedure itself usually takes 10-15 minutes and is performed under short-term general anaesthesia, which significantly saves time and burden for the patient," explains Július Örhalmi.

No special dietary or hygiene measures are required after the procedure. Doctors recommend a normal regimen with moderate rest for at least three days. However, as early as the second day, the patient can return to their daily activities and most are able to return to work soon.

High-tech and minimally invasive approach

At the Beroun hospital, doctors have the most advanced technical equipment and all the methods available in contemporary proctology. Most procedures are performed minimally invasively using laser surgery for all diagnoses or ultrasound-guided interventions for haemorrhoids without the use of an incision. Appointment times for the operating room range up to one month.

Safety and expertise first

As with any surgical procedure, certain complications can arise. However, Dr. Örhalmi assures that these are only a few per year. "Most often we encounter postoperative bleeding, which may require revision. There may also be pain or wound suppuration. However, these are really the exceptions; last year we performed approximately 500 procedures and only one patient had to be hospitalised for one day longer."

He prides himself on the high level of professionalism at his practice and, as the guarantor of the entire program, requires certification in coloproctology. "Proctology care at our practice is provided exclusively by board-certified coloproctologists or physicians who are actively completing their board certification. Currently, the team consists of four dedicated coloproctologists, and we pride ourselves on expertise and quality of care without compromise," says the physician.

Experience from abroad and a look into the future

According to Örhalmi, the future of the field lies in minimally invasive methods, which are still not the standard everywhere. "In many facilities, patients still undergo extensive operations with long hospital stays and healing times lasting several months. Our aim is to train as many proctologists as possible who will be able to introduce this approach in other regions. Therefore, we are also planning to start hands-on workshops with the possibility of transferring the surgeries anywhere in the world. Thanks to the technical background and expert guidance, we will soon start organizing training seminars for the professional public," adds the specialist, who was most influenced in his approach to modern proctology by his foreign internships in Barcelona and Bordeaux. He is also the head of the Coloproctology Centre at the hospital in Hořovice, where, among other things, he treats complicated fistulas using stem cells.
