Department of Aftercare

The inpatient follow-up care unit offers a well-equipped environment, with single- to four-bed rooms featuring modern adjustable beds and televisions. It provides follow-up care and long-term nursing, rehabilitation, and resocialization care for adults—primarily geriatric patients at risk of progressive deconditioning—who no longer require care in acute care inpatient units. It also provides geriatric consultations for inpatient units and primary care physicians. Patients are typically transferred to the unit directly from acute care beds, but this is not the only option—a patient’s family, primary care physician, certain social service agencies, etc., may request admission. An advantage of this department is its direct connection—both in terms of facilities and staff—to the hospital’s Department of Internal Medicine and Rehabilitation Center, as well as access to a wide range of specialized diagnostic tests. As part of its healthcare services, the follow-up care unit also provides basic physical therapy, therapeutic physical education, and basic occupational therapy, including training in the use of compensatory and prosthetic aids. For patients with aphasia, it offers communication training and speech re-education. The nature of all care depends on the degree of impairment of mental functions, the degree of deconditioning, and the individual need for specialized nursing care.
