Proctology is one of the fields that has undergone significant development in recent years. Thanks to minimally invasive methods and same-day surgery, patients can now undergo a range of procedures gently, quickly, and without the need for a long hospital stay. It is precisely this modern approach that is being actively developed by the team led by MUDr. Július Örhalmi, Ph.D., FASCRS, MBA, at the One-Day Care Center of the Rehabilitation Hospital Beroun and Hospital Hořovice.

A wide range of procedures without the need for a long hospital stay

Through the same-day surgery program, Dr. Örhalmi’s team performs nearly the entire spectrum of proctological procedures. The most common procedures involve the treatment of hemorrhoids, anal fissures, fistulas, and pilonidal cysts.

This type of care is particularly suitable for patients who prefer a short hospital stay, do not feel very comfortable staying in an inpatient ward, and want to return to work and their normal lives as soon as possible. Conversely, it is not ideal for patients with more serious comorbidities.

Simple preparation, a short procedure, and a return to normal routine

“Preoperative preparations are very straightforward. The patient fasts (no food or drink) starting at midnight, and in the morning, basic preparation—such as inserting a glycerin suppository—is sufficient. The procedure itself usually takes 10–15 minutes and is performed under short-term general anesthesia, which significantly saves time and reduces the burden on the patient,” explains Július Örhalmi.

No special dietary or hygiene measures are necessary after the procedure. Physicians recommend a normal routine with moderate rest for at least three days. However, as early as the second day, the patient can resume their daily activities, and most are able to return to work soon after.

Cutting-Edge Technology and a Minimally Invasive Approach

At the Beroun Hospital facility, physicians have access to state-of-the-art technical equipment and all available methods offered by modern proctology. Most procedures are performed using minimally invasive techniques, such as laser surgery for all diagnoses or ultrasound-guided procedures for hemorrhoids without incisions. Wait times for operating room appointments are typically within one month.

Safety and Expertise Come First

As with any surgical procedure, certain complications can arise. However, Dr. Örhalmi assures us that these occur in only a handful of cases per year. “The most common complication we encounter is postoperative bleeding, which may require revision surgery. Pain or wound infection may also occur. But these are truly exceptions; last year we performed approximately 500 procedures, and only one patient had to be hospitalized for one additional day.”

He prides himself on a high level of professionalism at his clinic and, as the program director, requires certification in coloproctology. “Proctological care at our clinic is provided exclusively by certified coloproctologists or physicians who are actively completing their certification. Currently, the team consists of four specialized coloproctologists, and we are uncompromising when it comes to expertise and the quality of care,” says the physician.   

International Experience and a Look to the Future

According to Dr. Örhalmi, the future of the field lies precisely in minimally invasive methods, which, however, are still not standard practice everywhere. “In many facilities, patients still undergo extensive surgeries involving long hospital stays and recovery periods lasting up to several months. Our goal is to train as many proctologists as possible who will be able to implement this approach in other regions as well. That is why we also plan to launch hands-on workshops with the ability to stream surgeries anywhere in the world. Thanks to our technical facilities and expert guidance, we will soon begin organizing educational seminars for the medical community,” adds the specialist, whose approach to modern proctology has been most influenced by his international fellowships in Barcelona and Bordeaux. He is also the manager of the Coloproctology Center at the Hospital Hořovice, where, among other things, he focuses on treating complicated fistulas using stem cells.
