Provision of rehabilitation care in the Beroun Hospital

Introduction

When Jessenia submitted the privatisation project for the Beroun Hospital in 2007, the primary focus of the hospital was on early and post-treatment rehabilitation, particularly in view of the fact that at the time of the submission the Central Bohemian Region did not have any early treatment rehabilitation beds. This plan is now being implemented by gradual steps, forced by the organisational and structural changes to the hospital.

Early inpatient medical rehabilitation is provided to patients after acute illnesses, operations and injuries by transferring them from bed to bed as soon as their health condition permits the full use of physiotherapy and rehabilitation medicine procedures (e.g. usually 7-10 days after surgery; for acute illnesses and injuries, the length of hospitalisation in an acute clinical unit varies according to the severity of the illness and the extent of the injury). Early inpatient medical rehabilitation is provided in departments of physiotherapy and rehabilitation medicine with acute inpatient care facilities in the facility, for health insurance companies the specialty of the department is 2H1.
Subsequent inpatient medical rehabilitation is provided to patients with long-term or permanent disabilities who have undergone a stage of early medical rehabilitation (either inpatient or outpatient), with a stable health condition (usually from 2-3 months after an injury, illness or surgery), whose health condition requires long-term rehabilitation care. It is provided in rehabilitation institutes or similarly specialised departments of some hospitals. This type of rehabilitation does not require the inpatient unit to be part of an acute inpatient care facility.

Present

At the time of privatisation, the Beroun Hospital had an outpatient physiotherapy department, which also provided physiotherapy in clinical beds. In May 2009, a temporary inpatient department of physiotherapy and rehabilitation medicine was established in Pavilion B, which was reconstructed in 2008 for the needs of the LDN, providing early medical rehabilitation. The department currently has 42 beds and outpatient operation. At the same time, it provides the necessary physiotherapy in the internal medicine and LDN beds. It has additional limited bed capacity for the needs of the Arthroscopy Centre, operating as a day care centre.

In May 2010, the Endowment Fund of MUDr. Jiří Besser, the current Minister of Culture, was established with the aim of revitalising the park of the Beroun Hospital and building a rehabilitation zone for the training of everyday movement activities of patients of the Department of Physiotherapy and Rehabilitation Medicine of the Beroun Hospital.

1st stage of reconstruction 2009-2010

Already in the autumn of 2009, the reconstruction of Pavilion D (originally the infectious diseases pavilion, then internal medicine and before privatisation the LDN and the headquarters of the Ambulance Service) into the first part of a complex early medical rehabilitation unit was started. By October this year at the latest, the Department of Physiotherapy and Rehabilitation Medicine will be moved to the reconstructed premises of Hall D, the LDN will be moved to Hall B and the reconstruction of Hall E will begin to form the next part of the medical rehabilitation centre. As part of the reconstruction of Hall D, the connection between Buildings D and E on all floors has been implemented. At this stage, the Department of Physiotherapy and Rehabilitation Medicine will have 37 beds (including 7 single rooms, all with private sanitary facilities) in Hall D and 12 beds in Hall C.

2nd stage of reconstruction 2010-2011

The focus will be on the redevelopment and refurbishment of Pavilion E. The ground floor of the pavilion will house the organisational and outpatient facilities of the department, the first two floors will have additional bed capacity and the necessary space for physiotherapy. On the top floor there will be a first bed capacity for inpatient rehabilitation, which will be added after the third phase of the extension to a final capacity of about 36-40 beds. On the first and second floors (connected to Building D), a further 30-35 beds for early medical rehabilitation will be added.
In this phase, the hospital park will be landscaped and a rehabilitation zone will be built (Figures 2 and 3).

Phase 3 of the 2011-2012 redevelopment

In this final part of the rehabilitation facility construction, Pavilion E will be expanded with an extension that will include a large hydrotherapy area including two rehabilitation pools, gymnasiums for group exercise and physiotherapy rooms for the care of inpatients, rooms for rehabilitation on machines, and two additional floors with single rooms that will have loggias in addition to their own sanitary facilities (Figure 1). The bed capacity will increase by an additional 32 beds.

Final state

In its final form, the rehabilitation unit of Beroun Hospital will provide early (80-90 beds) and follow-up (35-40 beds) medical rehabilitation in three interconnected buildings with the necessary physiotherapy and educational facilities and outpatient services. In the adjacent park there will be not only a rehabilitation zone for severely acutely physically disabled patients, but also other areas where people with disabilities and elderly citizens will find opportunities for rest and physical activity.
