Consent for hospitalisation

As part of your admission, you must sign a consent form. You will be asked to comment on certain circumstances of your hospital stay, such as providing information about your medical condition to loved ones, using an identification bracelet for secure identification, etc... To help you prepare, you can view the form here.

Consent for a procedure or surgery

For more complex diagnostic or treatment procedures, you will be asked to provide written consent on the Patient Informed Consent form. You will find general information about the operation or procedure, its benefits and also possible complications. A detailed explanation and the individual treatment procedure will be given to you by the attending physician.
