"Occupational therapists also focus on environmental adaptation and the selection of compensatory aids. The main goal of occupational therapy is to increase the functional independence and overall quality of life of patients. It is not only about cooperation with the patient, but we also try to involve the family and use all available means to enable their active participation not only in the home and work environment, but also in society," says Pavla Košková, head occupational therapist at the Rehabilitation Hospital Beroun. Occupational therapists are also part of various interdisciplinary and multidisciplinary teams, which are often specialised for patients with a particular type of diagnosis, and the composition and cooperation within the team depends on this. An example would be a neurorehabilitation team in which the occupational therapist works closely with the physiotherapist, clinical speech therapist, clinical psychologist, physician, nurse and nursing staff. Conversely, in other teams, collaboration may also be established with a prosthetist, nutritional therapist or special educator.
Robotically assisted therapy
"At Beroun Rehabilitation Hospital, we primarily use technology to support upper limb and cognitive rehabilitation. This includes robotic-assisted therapy, which helps patients perform precise movements that support the recovery of motor functions (e.g. a robotic glove that combines passive hand mobilisation with action-observation therapy). Furthermore, feedback systems are used to monitor and improve the patient's physical and cognitive performance through feedback," explains Kristýna Hoidekrová, PhD, coordinator for rehabilitation, science and research and occupational therapist at Beroun Hospital.
To support cognitive functions, the occupational therapists at Beroun also use special computer software to train memory, attention and other mental abilities. As part of science and research, transcranial direct current stimulation is used in occupational therapy to stimulate the brain and improve its functions.
Occupational therapy also for outpatients
A great positive of the Rehabilitation Centre of the Beroun Hospital is the involvement of occupational therapy in the complex care of patients, where every referred patient has access to occupational therapy examination and intervention. In addition to the occupational therapy provided primarily for patients at the Rehabilitation Centre, the hospital also provides care in the geriatric aftercare unit, where individual occupational therapy and group cognitive training take place. "We have newly started cooperation with the Mental Rehabilitation Centre, which is part of the complement of our hospital. Here we apply new approaches to the therapy of patients with cognitive disorders in occupational therapy, combining brain stimulation with special software for cognitive rehabilitation," says Kristýna Hoidekrová, adding: "In the future, we plan to combine this innovative approach with robotic rehabilitation and sensory therapy. In addition to occupational therapy for hospitalised patients, we also provide outpatient occupational therapy, which is not quite common in the Czech Republic."
This specialised outpatient care is mainly indicated for patients with acquired brain injuries, fractures and post-operative conditions in the upper limbs. Last but not least, occupational therapists are engaged in research and cooperate with a number of universities, thus supporting the development of this field not only in their workplace.
